Dear
Resident:
Welcome to St. Joseph’s Health Centre, Guelph, your new
home.
Our goal is to provide care that will meet your physical, intellectual,
social, cultural, spiritual, and emotional needs in a warm,
caring and home like atmosphere. We have a sincere interest
in the well being of every person and will do everything possible
to ensure your comfort and safety.
On the day you arrive, you may expect the following:
• Your photograph will be taken so that it can be used
for identification purposes.
• A nurse will meet with you to discuss your medical and
other health needs.
• Your family will be invited to join you for your first
meal while they help you to settle into your room.
• Our staff can also provide you with information about:
a. designating a Power of Attorney (to make decisions about
your personal care if – for some reason - you are unable
to do so for yourself); and,
b. making Emergency Care Plan (that is, making decisions about
how you would like to be treated if you were facing a life-threatening
situation).
• Laundry personnel with come to your room to label your
clothing
Finally, this booklet will give you some general information
about our programs and living arrangements.
