Individualized Life Planning
Your personal care needs are very important both to you and
to us, however, we recognize that there are other equally important
aspects of you and your life that make you unique. We want to
learn more about your personal history, life-long interests,
significant relationships and preferences. In this way, we are
better able to help you to live a dignified and meaningful life
in your new home at St. Josephs Health Centre.
About two weeks after you move in, you will be invited to a
meeting called Getting to Know You. Staff representatives
from various departments (Food & Nutrition Services, Nursing,
Recreation Therapy and Social Work) will meet with you and your
family to discuss your abilities, needs, preferences and routines
in order to develop a LIFEPLAN.
A doctor will be responsible for managing and prescribing your
medical care, the Lifeplan will have all of your
needs (physical, social, emotional, etc.) along with the details
of how, when, where and by whom each aspect of your care will
be provided. A copy is shared with the team working in the neighbourhood
where you will be living.
In three months, we will get together again to see how you are
adjusting and how we can be of further help. From that point
onwards, the staff will continue to meet with you and/or your
family on an annual (or as needed) basis to review and update
your Lifeplan.
If your needs change at any time, and you require admission
to another facility, or you wish to return home, our Social
Worker will assist you in making the necessary arrangements.
